a. Amended Disclosure Statement for Amended Joint Chapter 11 Plan (the "Disclosure Statement"). Exhibit A of the Disclosure Statement is the Amended Chapter 11 Plan (the "Plan").
b. Letter from the Trustee and the Official Committee of Unsecured Creditors recommending acceptance of the Plan.
c. Order approving the Disclosure Statement and setting the deadline to file ballots and the date of the hearing on confirmation of the Plan (Exhibit G of the Disclosure Statement).
e. Ballot to vote on the Plan. The ballot is BLUE. If your claim is in the name of more than one person each claimant must sign the ballot or the ballot will not count. If the claim is in the name of a corporation, trust, partnership, etc., please make sure that the person signing the claim indicates his or her title (i.e., president, general partner, trustee, etc.). PLEASE INDICATE YOUR VOTE (ACCEPT OR REJECT) AND SIGN YOUR BALLOT. UNSIGNED BALLOTS DO NOT COUNT. BALLOTS THAT DO NOT SHOW A VOTE DO NOT COUNT.
f. An envelope addressed to the FinFed Balloting Agent, ClaimTrack Services Group, P.O. Box 600488, Jacksonville, Florida 32260. Please put the BALLOT in this envelope and return to the FinFed Balloting Agent as soon as possible. Ballots received after 5:00 p.m. on June 4, 2001 will not count.
The claim amount you insert on the ballot is for voting purposes only. The Trustee reserves the right to object to any proof of claim you file for distribution purposes even if your vote is counted in the claim amount you have written on your ballot.
